MGMAT percentiles and scoring combinations

Hi I have 2 somewhat related questions:
1. Have the makers of the GMAT or MGMAT published a table showing what verbal/quant score combinations lead to which total scores?

2. I took an MGMAT CAT test yesterday and got a 45 on the quant. The score report says that would put me in the 68%, but table in the back of the 13th OG says a 45 would be in the 71% and the table in the green quant book says 75%. Why such a spread?
